Background

Malaysia Airlines is the flag carrier of Malaysia headquartered at Kuala Lumpur International Airport. The airline flies to destinations across Europe, Oceania and Asia from its main hub at Kuala Lumpur International Airport as well as a secondary hub at Kota Kinabalu International Airport. Malaysia Airlines Berhad is formerly known as Malaysian Airline System (Sistem Penerbangan Malaysia).Malaysia Airlines is a part of Malaysia Aviation Group, which also owns two subsidiary airlines: Firefly and MASwings. Malaysia Airlines also owns a freighter division: MASkargo.

Malaysia Airlines traces its history to Malayan Airways Limited, which was founded in Singapore in the 1930s and flew its first commercial flight in 1947. It was then renamed as Malaysian Airways after the formation of the independent country, Malaysia, in 1963. In 1966, after the separation of Singapore, the airline was renamed Malaysia–Singapore Airlines (MSA), before its assets were divided in 1972 to permanently form two separate and distinct national airlines—Malaysian Airline System (MAS, since renamed as Malaysia Airlines) and Singapore Airlines (SIA).[6]

Despite numerous awards from the aviation industry in the 2000s and early 2010s,[7][6] the airline struggled to cut costs to cope with the rise of low-cost carriers (LCCs) in the region since the early 2000s.[8] In 2013, the airline initiated a turnaround plan after large losses beginning in 2011 and cut routes to unprofitable long-haul destinations, such as Los Angeles, Buenos Aires and South Africa.[9] That same year, Malaysia Airlines also began an internal restructuring and intended to sell units such as engineering and pilot training.[9] From 2014 to 2015, the airline declared bankruptcy and was renationalised by the government under a new entity, which involved transferring all operations, including assets and liabilities as well as downsizing the airline.[10][11]

Now, About 9M-MTK

9M-MTK is the 11th Airbus A330-300 bought by Malaysia airlines on 15 Feb 2013. Although flying in a new livery [Negaraku livery], some say that Malaysia airlines will phase out the a330-300 one by one to make way for their new A330-900NEO.

Incidents

On the night of 18 July 2018, a Malaysia Airlines Airbus A330, registered 9M-MTK, took off on a regular public transport flight from Brisbane, Queensland to Kuala Lumpur, Malaysia. There were 14 crew and 215 passengers on board. Covers had been left on the aircraft’s three pitot probes (airspeed sensors). The instruments showed a red speed flag in place of the airspeed indication from early in the take-off, and unrealistically low airspeeds afterwards.

The flight crew did not respond to the speed flags until the aircraft’s speed was too high for a safe rejection of the take-off, and the take-off was continued. The flight crew’s initial radio announcement of an urgency situation was not heard by the air traffic controller.

The flight crew climbed to 11,000 ft and circled while performing troubleshooting and other procedures, which led to the shutting down the aircraft’s air data systems. Doing so activated the back up speed scale (BUSS), a safety function that displayed safe flight envelope information to the flight crew in lieu of airspeed. Using this system, airspeed management procedures, and assistance from air traffic control, the flight crew conducted an approach and landing at Brisbane.

For technical reasons, the main landing gear doors did not retract and were slightly damaged on landing. Also, nose wheel steering was not available and the aircraft remained on the runway for a short period before being towed to the gate.

The ATSB identified safety factors across a range of subjects including flight deck and ground operations, aircraft warning systems, air traffic control, aerodrome charts, and risk and change management.
