It seems that you don't actually have enough piston to retract for it to do a full rotation, try reducing the size some. I'm about to get into building piston engines, since I build mechanical stuff
I'm not too sure.... maybe try both and see what works.... maybe the throttle isn't set? Set the throttle to throttle. That's the problem. And set turn to rolling.

It works but you need another hinge on the back side of 2 pistons, so really 2 hinges. One for each side.
